The undersigned as President of the Business Men's fellowship USA Chapter identified below is duly authorized to execute this document on behalf of said Chapter and states that:

1. said chapter is under the general supervision of Business Men's Fellowship USA (hereinafter referred to as the "Fellowship"), and

2. said chapter authorizes the Fellowship to include it in the application to the Internal Revenue Service for the Group Exemption Letter, and

3. said chapter will promptly comply with all requests of the Fellowship for financial reports due at the end of each calendar year, and

4. said chapter will not act in any way contrary to the stated purposes of the Fellowship or in any way that might endanger the Fellowship's 501 (c) tax exemption, and

5. any failure to maintain proper accounting and financial responsibility in all chapter affairs will constitute a legitimate basis for the termination of the chapter charter.
